Adoption Application

     At SHELTIE RESCUE OF SOUTHERN MAINE we are looking for the best and last home for each of our dogs. We do not feel that “any" home is better than no home.  We are only looking for homes that:

• Treat their dogs like family members.

• Will keep this dog for the rest of the dog's life.

• Give their dogs the best in veterinary care.

• Insists on a fenced yard for safety.

• Keeps their dog safe at all times.

• Considers the dogs welfare in all decisions. 

• Will make time to train their dog.

• Will make time daily to spend with the dog.

• Will make sure this dog is a priority.

• Will make sure this dog never lacks for anything.

     Note:  Please take your time, and make sure you answer each question in detail.  This is our first introduction to you, and we are looking for complete, well-thought-out answers.  Please make sure you do not miss any questions, and make sure that all requested information is included.  Any missing information will cause your application to be rejected/denied.

     *****Applications that have missing answers will be considered incomplete will not be processed.

     *****Applications that have missing requested information/documents (i.e photos, vet reference letter) will be considered incomplete and will not be processed. 

     *****Applications with one-word answers, and no insight as to your thoughts, will not be processed.

Please note:   If you list one dog as your preference, and that dog gets adopted, we will not contact you.   If you list "any dog that needs me" then we will consider you for the dog that best matches your lifestyle.

WE RESERVE THE RIGHT TO REFUSE ANY APPLICANT, IF, IN OUR JUDGMENT, THE HOME SITUATION IS NOT COMPATIBLE WITH THE NEEDS OF A SHELTIE OR IF THE BREED IS NOT SUITABLE FOR THE APPLICANT.

WE RESREVE OUR RIGHT TO NOT DISCLOSE THE REASONS FOR REFUSING THE APPLICATION.

     All potential adopters regardless of limitation must be able to physically feed themselves and the dog unassisted, transport the dog to the vet & give physical activity & potty breaks outdoors within a safe environment.

*  Dogs with exercise needs must be able to be met by the adopter.

     As a matter of policy, SRofSME. does not discriminate on the basis of race, color, gender, sexual orientation, age, religion, national or ethnic origin, or physical disability.

INSTRUCTIONS:

• Once the "Adoption Application Form" loads to your screen, you should see a control panel pop up to download the form to your computer, OR "right click" on the form OR go to the "file" menu to SAVE the form to your computer, then. after downloading (saving) the form to your computer, fill out the form and save it, then print it, sign it and then scan it and then email it to us.
